UC SHS Reservation Guidelines for SY 2021-2022

- For Senior High School
- Accomplish the Admission Form:
a. Log on to UC SHS Admission using any web browser, OR
b. Download the form and email accomplished form to [email protected]
- Pay reservation fee of Php 2000.00*
- Cashier Office, UC Main Campus
- Bank
- Online Banking thru Bank's Mobile App or Bank's Website
- Over-the-Counter using Pay Slip/Bills Collection Slip
Bills Payment is available at the following banks
Biller/Company Name: University of the CordillerasSubscriber's Name: 'Student's Full Name'Subscriber's Number: 'Student's ID Number'
* Non-Refundable
- Email proof of bank payment with the student's name to [email protected]. A confirmation email shall be sent along with the student's Official Registration Form where the student's ID Number shall be indicated.
- Form 138 (Grade 10 Card) with Learner Reference Number
- Copy of Birth Certificate (PSA/NSO)
- ESC/QVR Certificate (for Private JHS completers)
Note: Other admission requirements shall be submitted within the first trimester of the school year.
